COVID-19 Guidelines Update:
We are looking forward to seeing you during your upcoming appointment. In preparation, we ask that you carefully read through the updates to our protocols for on-site appointments. It is critical that you follow the instructions provided so that we can safely treat you at your scheduled time. We want you to know that we are taking every precaution to ensure your safety as well as that of our employees. Please do not hesitate to let us know if you have any additional questions or concerns.
You will receive a notification for your confirmed appointment at the time of booking. Within that email was a request to visit our Patient Portal and complete your pre-appointment paperwork. Please ensure that you have filled out all questionnaires and consents prior to arriving at our practice. If you are unable to complete them prior to your arrival, we will ask that you log into the Patient Portal via your mobile device and complete them before we can see you.
Within 24-Hours of Your Appointment
Within 24 hours of your appointment, you will receive a COVID-19 related pre-screening questionnaire & consent for treatment inside of your Patient Dashboard. It is imperative you complete both the questionnaire and consent prior to entering our practice. We are available by phone should you have any questions regarding either one.
Please arrive 10 minutes prior to your appointment, in case there is an issue with your documents. We look forward to seeing you soon!
- When you arrive in our parking lot, please respond to your Appointment Reminder text to let us know you've arrived.
- Please remain in your vehicle until we notify you to enter the practice for your appointment time.
- Upon entering, please make immediate use of our hand sanitizing stations throughout the practice.
- We ask that you wear a mask into our practice; however, it may be removed to perform your treatment.
48 Hour Cancellation Policy
Please be aware that we require a non-refundable $100 deposit to book your appointment with Dr. Castelli. The deposit can be applied toward any service for up to one year. However, if you cancel/reschedule within the 48 business hrs, you will forfeit your deposit and will need to pay again to reschedule your appointment. The deposit will be credited toward any non-surgical procedure for up to one year.
We recommend leaving your valuables at home as we’re not responsible for any lost or stolen personal items.
